Washington, D.C. — April 22, 2026 — Advocates for Community Health (ACH), a national leader in advancing federal policy and driving innovation to strengthen community health centers, today announced a new multi-year partnership with Aledade, a physician-led public benefit corporation and national leader in value-based care. Through this collaboration, ACH is expanding the tools, resources, and strategic support available to its members to continue to help community health centers (CHCs) deliver high-quality, sustainable, and patient-centered care.

A cornerstone of the partnership is the introduction of a new ACH member benefit: access to Aledade’s accountable care organization (ACO) model, designed to help health centers thrive in value-based care arrangements. This opportunity gives ACH members the ability to participate in an ACO composed of like-minded organizations.

“ACH is proud to have Aledade’s support as we strengthen benefits for ACH members,” said Amanda Pears Kelly, chief executive officer of ACH. “Aledade’s leadership in value-based care and data-driven support for providers helps ensure that health centers can continue to innovate while staying focused on what matters most – delivering accessible, high-quality care to millions of patients.”

Aledade empowers clinicians to improve patient health, grow thriving primary care organizations, and share in the rewards of better care. Aledade serves over 3,000 primary care organizations, including over 370 CHCs, that care for more than 3 million patients in value-based care programs across 46 states and the District of Columbia. Through its ACOs, Aledade provides actionable insights, AI-driven technology, personalized coaching, and policy expertise to help clinicians improve outcomes and achieve the shared savings that support business sustainability. In 2024, Aledade’s 10 CHC-only ACOs earned a total of more than $96 million in shared savings and achieved over $585 in savings per beneficiary.

As part of the collaboration, ACH members will gain access to:

  • Participation opportunities in Aledade-managed ACOs
  • Advanced data insights and performance analytics
  • AI-enhanced tools to improve patient care and operational efficiency
  • Dedicated support and coaching to navigate value-based care workflows
  • Expertise in policy and contracting across 200+ value-based arrangements

“Aledade welcomes the opportunity to support the vital work of community health centers across the country,” said Mat Kendall, co-founder and president of Aledade. “We are dedicated to providing advanced technology, support, and insights that empower clinicians to prioritize patient care. By joining an Aledade-managed ACO, health centers can be rewarded for their commitment to delivering high-quality patient care and improving patient outcomes.”

Together, ACH and Aledade aim to strengthen the long-term sustainability of community health centers while advancing accessible, high-quality care for nearly 34 million patients across the country.
