On June 17, 2024, Advocates for Community Health (ACH) launched Phase 2 of the Community Health Entrepreneur Challenge under our ongoing, multi-year collaboration with UnitedHealthcare Community & State. To date, this collaboration has awarded $2.5 million in grant funding to support community health centers across the U.S.
ACH’s Director of Programs & Engagement Liezl Perez Schewe has compiled a list of frequently asked questions below that may be of interest to ACH members.
Can we submit more than one proposal?
No, we will accept only one submission per health center.
Can current Entrepreneur Challenge grantees apply for this new seed funding?
No, the community health centers that won last year are ineligible to apply.
What are the requirements?
Award recipients must be ACH members. They will be required to submit quarterly and final reports, attend virtual and in-person convenings as necessary, and present a summary of findings at the ACH Annual Member Meeting or other appropriate learning events when applicable.
What is the key focus area for the proposal?
ACH members are welcome to submit one technology-related proposal.
How will my proposal be evaluated?
The proposals will be evaluated by a review committee who will select two awardees. Applications will be scored on a scale of 0 to 100 points based on the criteria outlined below.

Can I submit a supporting document with my proposal?
Yes, we encourage you to provide supporting documents such as a project work plan and timeline; budget, including a detailed justification of expenses; statement of Community Health Center Commitment (not to exceed one page); bibliography; appendix (e.g., letter/s of support, awards and recognition, glossary of terms, etc.).
Send a file of no more than 10 pages in Word or PDF format directly to info@advocatesforcommunityhealth.org by Monday, July 22.
Can the application deadline be extended?
Applications will only be accepted until Monday, July 22. The review committee needs enough time to review all applications and proposals.
When will I find out the results?
We will directly reach out to the two grant awardees in mid-August via email.
